Miami County Health Department is asking all Miami County residents to complete a short community health survey. 

The goal is to understand how many people in Miami County have or do not have symptoms of coronavirus (COVID-19). Please accurately complete this short survey only if you are 18 years old or older and live in Miami County.

We would like for a survey to be completed for every member of your household. Please complete a survey for yourself. At the end of the survey, you can will have the option to complete additional surveys on behalf of other members of your household. Information provided will only be shared with the Miami County Health Department and Miami County Medical Center.
